RA appointment procedures

To complete your RA application, please read the instruction below and submit the necessary documents.
If you don't receive your salary, you don't have to submit these documents.

Dead line: No Later Than 9th April, 2012.

◆1. Appointment_File.xls (Excel file. please check and fill out 2 sheets)
* All members need to submit

How to submit:
Please submit by mail and name the file: “Your name (FAMILY First name ) - Appointment”
* Your address must be the same as alien registration card.

◆2. Salary_Sheet.pdf
◆3. Deduction_for_Dependents_Sheet.pdf

*Those who are a new GMSI RA must submit 2 and 3.
*Those who were a GMSI RA in 2011 and correspond to the below must submit 2 and 3.

・If your bank account has changed.
・If you submitted the Deduction for Dependents Sheet in December last year to the GMSI office, and if any of your information has changed. (example: Name, address, marital status)

How to submit:
Please Submit hardcopy (signature or personal seal required)
An example is attached so please check.

NOTE!
If you do not correspond to the above, please inform us that you will not submit 2 and 3 when submitting the 1. Appointment File.

≪For international students only≫
International students must also submit:
* If your account number doesn't change, you don't have to submit it.

◆4. A copy of the cover of your bank book (the page on which your name is printed)

Please contact the GMSI office if-for any reason-you are unable to submit the required forms before the deadline.
