In accordance with the upcoming end of your delegated appointment period as a research assistant with this GCOE program on March 31, 2012, please submit the following materials.

  (This format is for both, Completion and Application. Please use this form
  if either applies to you.)
2011 Report on Research Results.doc
Contact List
  (If you are applying for the GMSI RA again, you do not need to submit
  this form.)

【How to submti】
◆①② ⇒ It must be checked by your Advising Professor, and sent to the address below.
    <GMSI office address>:
    GMSI office, Room 203 (2nd floor), Engineering Bldg. 2, Hongo Campus
    Ms. Terunuma, Ms. Enomoto

◆③④ ⇒ The student must prepare it and send it to the GMSI office by mail.
    <Mail address>: gmsi-ra-registration@mechasys.jp
    ③Change the name to「H23result-専攻名-氏名」 and send it by word file.
    ④Change the name to 「H23contact-専攻名-氏名」 and send it by excel file.
