  (If you are applying for the GMSI RA again, you do not need to submit
  this form.)

◆①② ⇒ It must be checked by your Advising Professor, and sent to the address below.
    ＜GMSI office address＞：
    GMSI office, Room 203 (2nd floor), Engineering Bldg. 2, Hongo Campus
    Ms. Terunuma, Ms. Enomoto

◆③④ ⇒ The student must prepare it and send it to the GMSI office by mail.
    ＜Mail address＞： gmsi-ra-registration@mechasys.jp
    ③Change the name to「H23result-専攻名-氏名」 and send it by word file.
    ④Change the name to 「Ｈ23contact-専攻名-氏名」 and send it by excel file.
